Alpecin or minoxidil: which works better against hair loss? 6 differences in a row

Are you unsure between Alpecin and minoxidil against hereditary hair loss? Short answer: minoxidil 5% has the strongest evidence. In a double-blind study with 393 men, minoxidil gave 5% significantly more hair growth after 48 weeks than a placebo[1]. For caffeine products like Alpecin, the evidence is thinner: a 2025 systematic review found 9 small studies, 5 of which had very low-quality evidence[2]. Alpecin is much cheaper and has few side effects. In this comparison we list the 6 most important differences, honestly and with sources, so you can choose for yourself.

What is Alpecin and how would caffeine work?

Alpecin is a German brand from Dr. Kurt Wolff who is best known for the Caffeine Shampoo C1. According to the manufacturer, caffeine strengthens weakened hair roots and the caffeine complex reaches the hair root within 2 minutes. That is why Alpecin recommends using the shampoo daily and allowing it to work for 2 minutes[3]. In addition to the shampoo, there is the Caffeine Liquid, a tonic that you leave on the scalp[4].

Minoxidil works differently. It is a lotion or foam that you apply to the thinning areas twice a day and let it sit[5]. According to Apotheek.nl, minoxidil on the skin causes less hair loss, and sometimes new hair will grow.[6]. Minoxidil 5% has an official registration with package leaflet in the Netherlands and is only intended for adults aged 18 and over[5].

What are the 6 differences between Alpecin and minoxidil?

Below we compare Kirkland minoxidil 5% lotion with the two best-known Alpecin products on points that you can check yourself. Alpecin prices are the recommended prices on the Dutch Alpecin website (October 2026).

Is Alpecin as good as minoxidil, as a study says?

Alpecin likes to refer to one study: a study in India with 210 men, in which a caffeine fluid of 0.2% was "no less good" than minoxidil 5% after 6 months. The proportion of hairs in the growth phase increased by 10.6% with caffeine and 11.7% with minoxidil[7]. That sounds convincing, but note three points:

  • It was about the Liquid, not the shampoo. The liquid remains on the scalp. Rinse out a shampoo after 2 minutes.
  • It was an open study. Participants and researchers knew who used what. The authors themselves write that this can lead to distortion[7].
  • The manufacturer contributed to the payment. The study was financed by Dr. Kurt Wolff, and one of the authors works for this company[7].

The 2025 systematic review concludes that caffeine products appear safe and may help, but that better-designed studies are needed and that a good result is not guaranteed[2]. In 2018, the British advertising watchdog ASA also banned an Alpecin advertisement with the claim that the shampoo can reduce hair loss, because according to the ASA the claim was insufficiently substantiated[8]. Minoxidil 5%, on the other hand, was tested in double-blind studies against a placebo, in which it clearly worked better than 2% and than placebo[1]. Can you use Alpecin and minoxidil together?

Yes, that is in principle possible. A shampoo and a lotion that you leave on do not conflict: you wash your hair with the shampoo of your choice and then apply minoxidil to the thin areas, in the morning and in the evening[5]. There are no studies that show that the combination works better than minoxidil alone. So see Alpecin as a pleasant shampoo, not as a replacement for your minoxidil.

Do you have a sensitive scalp? Then the combination of a daily shampoo and minoxidil can sometimes irritate too much. Then choose a minoxidil with less alcohol, and do not use minoxidil on an inflamed, irritated or painful scalp. flakes.[5].

Good to know before you start minoxidil starts: in weeks 2 to 6 you may temporarily lose more hair. That's part of it and usually stops within a few weeks[5]. Stop and contact a doctor if you experience chest pain, fast or irregular heartbeat, dizziness, swollen hands or feet, or sudden weight gain[5]. Do not use it if you are pregnant[6]. If you stop taking minoxidil, the new hair may fall out again within 3 to 4 months[5].

Frequently asked questions

Does Alpecin really work against hair loss?

Maybe a little, but the evidence is weak. Most studies on caffeine products are small and some do not have a control group. The British ASA found the claim that the shampoo reduces hair loss to be insufficiently substantiated in 2018.

Is Alpecin Caffeine Liquid as good as minoxidil 5%?

One open study, paid for by the manufacturer, found a comparable improvement in the proportion of growing hairs after 6 months. That is one study with limitations. Minoxidil 5% has been tested against placebo in several double-blind studies.

How long do I have to use Alpecin or minoxidil before I see anything?

According to Alpecin, failure noticeably decreases after about 3 months. With minoxidil, according to the package leaflet, you will see results after 2 to 4 months. If you do not see any new hair growth after 4 months, stop according to the package leaflet.

Can I use Alpecin shampoo if I use minoxidil?

Yes. Wash your hair with the shampoo, dry it and then apply minoxidil to the thin spots. If you experience a lot of itching or redness, choose a milder shampoo or a minoxidil with less alcohol.

Is Alpecin or minoxidil suitable for women?

The caffeine study against minoxidil was only done on men. Minoxidil 5% foam for men is not intended for women according to the package leaflet; There are separate products for women, such as 2% lotion or 5% foam for women. If in doubt, consult your doctor or pharmacist.
